This is an opportunity to work in a specialist Social Work post within the Community Mental Health Team.
Main duties will include working with individuals with Mental Health problems, and supporting their families and carers. To provide the best possible service outcomes for the client group and support them to reach their full potential and promoting their independence within the local community.
The post holder will be working closely with Psychiatric wards and teams to help with hospital discharges.
To fulfil statutory functions as laid out in the Social Services and Wellbeing Act (Wales) 2014 and other relevant legislation pertaining to Mental Health.
It is an exciting opportunity for the successful applicant to contribute with the implementation of the Porth Ceredigion Integrated Through Age Services Model.
This vacancy requires a full UK driving licence and must have access to a car.
